• p_establishment_id : Establishment Id
• p_person_id : Person Name
• p_contact_name : Contact Name
• p_contact_telephone : Contact Telephone
• p_fax : Fax number
• p_email_address : Email Address
• p_dads : Include to initial DADS
• p_pmf5 : First recruit in Establishment
• p_date : Run Date
• p_batch : Receipts sent in batch
• p_acknowledgement : Type of acknowledgement for receipts
